In the U.S., check compliance with FDA 21 CFR Part 1271. Products that qualify under Section 361 (HCT/Ps) must meet two criteria: minimal manipulation and homologous use. If a provider offers lab-expanded cells, systemic IV infusions for general wellness, or claims to cure conditions like Parkinson's or diabetes without an active FDA Investigational New Drug (IND) authorization, they are operating outside federal guidelines. Ask directly whether the product is regulated under Section 361 (tissue) or Section 351 (drug). A provider who cannot answer that question clearly does not understand their own regulatory position.

02 — Biologic quality & lab verification

Biologic Quality & Lab Verification

Request the Certificate of Analysis for the exact lot being used, issued by an independent CLIA-certified laboratory. A meaningful CoA verifies sterility testing, endotoxin testing, communicable disease screening, and cell viability measured close to the point of use. A general marketing brochure about product quality is not a CoA, and a provider who cannot produce one with a verifiable lot number is asking you to take the most important part of the procedure on faith.

Clinical Standards & Delivery

Board certification in a relevant specialty such as interventional orthopedics, physical medicine and rehabilitation, or sports medicine, along with documented fellowship training or specialized certification in image-guided procedures. For complex joint or spine work, a general practitioner or mid-level provider without interventional training is a meaningful gap in the standard of care.

04 — Cost, marketing & ethics

Cost, Marketing & Ethics

Cost varies widely by procedure type, complexity, biologic used, and location. In the U.S., image-guided orthobiologic procedures such as autologous BMAC or high-concentration PRP commonly fall in the range of a few thousand dollars per joint area. International programs are structured differently and often bundle multiple components into a single price. The number itself matters less than whether the provider will tell you clearly what is included, what is not, and what could be added later.
