Data Deletion Instructions

This page explains how to ask Aurenza Health, LLC ("Aurenza Health," "we," "us") to delete personal information we hold about you, including information we receive through Facebook, Instagram, or other Meta products. It applies to our website, aurenzahealth.com, and our patient and staff portal, portal.aurenzahealth.com.

1. Who This Page Is For

This page is for anyone whose information we may hold, including website visitors who filled out a form, current and former patients, people who receive our texts or emails, and staff members who connected a Facebook or Instagram business account to our portal.

2. Information We May Hold

Depending on how you have interacted with us, we may hold:

  • Contact details you gave us, such as your name, email address, and phone number.
  • Messages you sent us by form, email, or text.
  • Your text and email preferences, including any request to stop receiving messages.
  • Patient portal account details and the health information that is part of your medical record.
  • For staff who connect a Meta business account to our portal: the Meta user ID and name linked to that connection, the access key Meta issues to our app, and advertising performance figures for Aurenza Health's own ad accounts.

We do not use Facebook Login for patients, and we do not send patient health information to Meta.

3. How to Request Deletion

Email info@aurenzahealth.com with the subject line "Data Deletion Request." Please include:

  • Your full name.
  • The email address or phone number you used with us.
  • What you would like deleted. You can ask us to delete everything we are allowed to delete, or name specific information.
  • If your request is about a Facebook or Instagram connection, the name on that Facebook account.

We do not charge a fee to handle a deletion request.

4. Removing a Facebook or Instagram Connection

If you connected a Facebook or Instagram account to our portal, you can remove our app's access yourself at any time:

  1. Log in to Facebook.
  2. Click your profile picture, then Settings & privacy, then Settings.
  3. Open Business integrations (or Apps and websites).
  4. Find "Aurenza Portal" and click Remove.

Removing the app stops any new data from reaching us. To also delete the data we already received, send the email described in Section 3. Once we receive it, we delete the Meta user ID, name, and access key linked to your connection, and we disconnect the ad account from our portal.

5. Stopping Texts and Emails

To stop text messages, reply STOP to any message from us. To stop emails, click the unsubscribe link at the bottom of any email. We keep a record that you opted out so we do not contact you again by mistake. If you also want your contact details deleted, send a deletion request as described in Section 3.

6. How We Verify Your Request

To protect you, we confirm that a request really comes from you before we delete anything. We may reply to the email address or phone number we have on file, or ask you to confirm a few details you have already given us. We will never ask for your password. If we cannot verify your identity, we will tell you why and what we need.

7. What Happens Next

We will confirm that we received your request. We will complete it within 30 days of verifying your identity, and we will tell you in writing when it is done. If we need more time, we will tell you why and when to expect it. Copies stored in our secure backups are removed automatically as those backups expire on their regular schedule.

8. Information We May Be Required to Keep

Some information cannot be deleted on request because the law requires us to keep it, or because we need it to protect you and others. This includes:

  • Medical records, which we must keep for the period required by applicable federal and state law.
  • Billing, payment, and tax records.
  • Records of your consent and your requests to stop texts or emails.
  • Security and access logs that protect our systems and your information.
  • Information we need to resolve a dispute or respond to a legal claim.

When we cannot delete something, we will tell you what we kept and why. We will stop using it for any purpose other than the reason we must keep it. If you are a patient, you can still close your portal account and stop receiving messages from us.

9. Our Service Providers

We use trusted service providers to host our systems and send our messages. Where those providers handle health information, they have signed agreements with us that require them to protect it. When we delete your information, we also delete it from, or instruct deletion by, the service providers who hold it on our behalf, unless the law requires it to be kept.

10. Your Other Privacy Rights

This page covers deletion only. For how we collect, use, and protect your information, please see our Privacy Policy. Patients can find their rights under federal health privacy law, including the right to access and request changes to their medical records, in our Notice of Privacy Practices.

11. Changes to This Page

We may update this page from time to time. When we do, we will change the effective date at the top.
