Chronic wounds, and diabetic foot ulcers above all, are where cell therapy research produces its most consequential endpoint: limbs. Multiple meta-analyses of randomized trials now report on healing rates and amputation, and the numbers deserve to be read carefully, in both directions.
Here is what they show, the certainty caveats that belong beside them, and why urgency changes everything in this category.

What the Research Shows
Chronic wounds, and diabetic foot ulcers above all, are where this research produces its most consequential endpoint: limbs. The randomized literature here is deep enough to pool many times over, and the numbers repeat across independent teams.
A 2023 meta-analysis of 19 randomized controlled trials covering 766 patients with diabetic foot ulcers found substantially higher healing odds with cell therapy, markedly lower amputation odds, and longer pain-free walking distance. A meta-analysis of eight randomized studies found an amputation relative risk of 0.25 and a wound healing relative risk of 2.05 in favor of cell therapy. An earlier seven-study meta-analysis covering 224 patients found partial or complete healing in 77.4% of cell-treated patients versus 31.9% of controls, and a pooled analysis of cellular therapy trials found better ankle-brachial index, transcutaneous oxygen pressure, healing rates, amputation-free survival, and pain scores.
The signal has held as the literature matured: a 2024 GRADE-compliant bootstrapped meta-analysis of randomized clinical trials again found cell therapy significantly superior to conventional approaches, and a 2026 pooled analysis of 32 randomized trials covering 2,059 patients now exists on ulcer healing and amputation outcomes.

The Honest Line
Numbers that strong deserve their caveats in the same breath. The underlying trials vary in cell type, dose, delivery, and wound severity, several are small, and certainty ratings across outcomes run from very low to moderate. A 2025 meta-analysis of six randomized trials found the healing benefit concentrated in smaller ulcers, with no significant benefit in large ulcers and no significant amputation difference in that particular analysis. So the fair summary is a consistent, repeated randomized signal that has not yet reached top-tier certainty. No study shows a guarantee it works every time, or that it will work for you, and every trial added cells to aggressive standard care, offloading, debridement, infection control, and vascular assessment, never in place of it. A wound that is infected, deep, or on a cold foot needs local care now, not a program with travel dates, and we will tell you so.
