What NAD+ actually does

NAD+ stands for nicotinamide adenine dinucleotide. It is not a vitamin or a hormone. It is a coenzyme present in every living cell, and it has two distinct roles that are worth separating.

The first is metabolic. NAD+ is a carrier in the reactions that convert nutrients into usable cellular energy. It shuttles electrons through glycolysis, the citric acid cycle, and oxidative phosphorylation in the mitochondria, cycling between its oxidized form, NAD+, and its reduced form, NADH. Without adequate NAD+, the machinery that produces ATP cannot run at capacity.

The second is regulatory. A family of enzymes called sirtuins, and another called PARPs, consume NAD+ as a substrate. Sirtuins are involved in mitochondrial quality control and metabolic regulation. PARPs are involved in DNA repair. Both draw down the same NAD+ pool, which means the pool is not only fuel for energy production but also the currency for cellular maintenance.

Why mitochondria are the focus

Mitochondrial efficiency sits at the center of current longevity science, and for a defensible reason.

Mitochondria produce the overwhelming majority of cellular ATP. They also accumulate damage over time. When damaged mitochondria build up faster than they are cleared, cells produce energy less cleanly, and the effect compounds across every tissue that depends on them, which is essentially all of them.

Cells have their own housekeeping system for this. Mitophagy is the process by which damaged mitochondria are identified and cleared. Mitochondrial biogenesis is the process by which new ones are built. Both are regulated by pathways that depend on NAD+ availability.

Why intravenous

Oral NAD+ precursors, including nicotinamide riboside and nicotinamide mononucleotide, are widely available and have been studied.

Intravenous delivery bypasses gastrointestinal absorption and first-pass hepatic metabolism, achieving different plasma concentrations than oral routes. That is a pharmacokinetic statement rather than a claim of superiority in outcome, and the comparative clinical question remains under study.

What IV administration does mean practically is that dosing is controlled, delivery is observed, and the infusion happens under clinical supervision as part of a coordinated program rather than as an unmonitored supplement regimen.
